FOR THE NEW HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT (HIPAA)
We
have drafted a new Health Insurance Power of Attorney that includes the
necessary HIPAA waiver language required by law. This will allow the family
members or friends you have named in your Health Care Power of Attorney
to act on your behalf without being questioned or challenged. Without
the HIPAA language, your health care providers will not disclose medical
information to your health care agent.
To
better prepare our clients for their initial consultations with us, we

To better prepare our
clients for their initial consultations with us, we provide the following

to organize your thoughts and information for the estate planning process.

files are all in PDF format, which requires Acrobat Reader to view/print.
Most computers already have this program installed, which you can check
by going to the START menu and looking the PROGRAMS popup list. If you
do not have it, you can download Acrobat
Reader for free by clicking on this link.
Viewing
these PDF Files
You can view these files in a couple ways, depending on how your computer
is configured. The more recent versions of Acrobat Reader include a "browser
plugin" which lets you simply click on the links below to open/view
the files right in your browser window. If you click the link below and
do not see the files, then your computer does not have the proper plugin
to do this. In that case, you can simply right-click on the files and
select ""save file as" or "download link to disk"
from the popup menu. This will save a copy of the PDF file to your computer,
which you can then locate and open using Acrobat Reader software.
